That you're aware of the OSHA protocols and requirements.
And that as "part of an ISO certification," your job was to insure those multiple pipe yards were in compliance.
However... (you knew that was coming, right!)...
Why am I not surprised that you assume that just because those are the standards... that all companies adhere to and follow them?
In this case, the article stated that OSHA found a "wholly inadequate safety culture" at that Amazon center. And, that they had "serious concerns" about safety training there; in that staff didn't participate in emergency drills, with some not even knowing the location of the designated shelter area. And the warehouse did not have an Emergency Action Plan, customized for the location, as they are required to have, nor did they properly follow the one they did have.
So, while it appears you have ultimate faith in rules, regulations, and the way things should be... and you certainly expect individuals to be accountable to them.
You also apparently fail to realize (or at least acknowledge) that individuals are not the only entities that fail to have that integrity!
